    man i woke up this morning with a toothache, worst pain ive had in my life next to a couple broken bones, i felt like i had been chewing on tin foil while i was sleeping, my wisdom teeth are coming in i’m awaiting my extraction next monday, and obviously i don’t have a choice, i’ve stayed awake for 2 whole days in pain and whenever i wake up in the morning it comes back

    been taken pain killers for 5 years now for L1-L2 back probs. At first the hydrocodone made me en depressed when i was coming down i got all sad and . but that was only for the first year, yeah i know first year. After that i messed my back up now im on Avinza (morphine) and Oxycodone. I am fine i often go off my meds for 1-2 weeks easy no withdraws what so ever. Got vaction coming up havnt had any in 2 weeks need them only when i work alot.

    i have back pain and was taking tramadol besides the fact it is ineffective for me in low doses. taking a high enough dose to notice any pain relief turns me into to psyco er. i feel like I’m going to rip someones face off for no reason. i hated that feeling so i quit taking it.

    in cause Paracetamol (my old Comment). I´am very glad about the new decision since 1/04/2009 (Germany). Not anymore without a prescription over 10g a unit:-)

    What would you advise to do for someone who has a back problem and is addicted to vicodin, takes 5 morphine (15 mg) pills unsubscribed and gets high on marijuana in the same day…could a person like that who has a chronic back problem (si joint) be admitted to a psychiatric place for just for taking drugs? Is that considered a psychiatric problem? I don’t mean to sound dumb, but there’s a person I’m close to that does this kind of thing on a weekly basis and people are worried about an overdose

    Mark, Sometimes they are needed but all are “nasty” Dugs. I belive Tramadol is less dangerous. Most addicting are medications like Pethidin or other “old” Opioides:-) I like Metamizol; NON toxic very effectiv and NO interaction with others or alcohol. One of most dangerous/toxic Drugs without a prescription is Paracetamol (probably the Barbiturate under the pain-Killers)
    A tricyclic is also very often helpful too
    But sometimes really needed (opioides). as Ultima ratio.

    Tramadol is an ssri antidepressant? I was recently Rx’d the stuff and I don’t know what to make of it, yet…I’m being pretty cautious. Great vids.

    Doc, what are your thoughts on Soma (carisoprodol) – I’ve had 3 back ops and hopefully will get SSDI because I just can’t work…Soma makes me a moron, but it’s been invaluable for spasms and lets me sleep – thanks in advance.

    i dont really like suboxone.. some opiate like meds can interact terribly with ssri antidepressants, example tramadol, do to its kappa receptor activity it acts like an ssri so no mixes with say maoinhibitor or severe reaction can occur. Yes it blunts the response to the antidepressant but is so common in practice a psychiatrist has to treat the depression even in the setting of this substance related disorder too, its tricky

Do you know if they have any effect on the efficacy of anti-depressants, as the narcotic meds do? I know that bup is a partial agonist, but wasn’t sure if it can be used in conjunction with anti-depressants. thanks.

    they make you numb, emotionally numb…. and the antidepressants dont work as well clincially I see this alot. its just the way it is. it blunts the response to antidepressants

    Yes they go thru liver meds like acetinominophen or ibuprofen mixed with hydrocodone in one pill are nortoriously bad on the liver. Cirrhois is also caused by alcohol and other things as well. Thanks
